You are an intern in A&E. A 28-year-old man is brought in having been assaulted. He has significant facial injuries. While examining him, he tells you confidentially that the assault was carried out by a named individual who he knows, and that he is 'absolutely certain' this person has attacked others and will do so again. He explicitly says he does not want the Gardaí involved.

A 55-year-old man with CKD stage 3b (eGFR 38 ml/min/1.73m²), hypertension, and a urine ACR of 8 mg/mmol requires antihypertensive treatment to slow CKD progression. Which blood pressure target and medication choice is most appropriate?

A Target BP <130/80 mmHg with an ACE inhibitor or ARB as first-line.
B Target BP <180/100 mmHg: a higher threshold is safer in CKD.
C Use a dihydropyridine calcium channel blocker only; ACE inhibitors are contraindicated in all CKD patients with reduced eGFR.
D Avoid all antihypertensives in CKD until eGFR falls below 20 ml/min/1.73m².
E Target BP <150/90 mmHg with any antihypertensive agent as first-line.
Answer Breakdown
✓ CORRECT
A. Target BP <130/80 mmHg with an ACE inhibitor or ARB as first-line.
NICE NG203 recommends a BP target of <130/80 mmHg in proteinuric CKD (ACR ≥3 mg/mmol). ACE inhibitors and ARBs reduce intraglomerular pressure by dilating the efferent arteriole, slowing CKD progression independently of their BP-lowering effect.
✗ INCORRECT
B. Target BP <180/100 mmHg: a higher threshold is safer in CKD.
Far above guideline recommendations; sustained hypertension at this level causes progressive renal and cardiovascular damage.
✗ INCORRECT
C. Use a dihydropyridine CCB only; ACE inhibitors are contraindicated in all CKD patients with reduced eGFR.
This reverses the guideline. ACE inhibitors and ARBs are specifically recommended first-line in proteinuric CKD. CCBs are appropriate add-on therapy when BP targets are not met with ACE inhibitor or ARB alone.
✗ INCORRECT
D. Avoid all antihypertensives in CKD until eGFR falls below 20 ml/min/1.73m².
Withholding treatment until eGFR falls to 20 would cause catastrophic hypertensive renal and cardiovascular damage. Hypertension must be treated promptly in CKD.
✗ INCORRECT
E. Target BP <150/90 mmHg with any antihypertensive agent as first-line.
A target of <150/90 mmHg is insufficient for proteinuric CKD; guidelines recommend <130/80 mmHg when ACR is elevated. Choice of agent also matters: ACE inhibitor or ARB first-line, not any agent.
◆ Key Distractor (C): ACE inhibitors do transiently reduce eGFR on initiation by lowering intraglomerular pressure. A fall of up to 30% is expected and acceptable. Their long-term renoprotective benefit far outweighs this.
◆ Stem Discriminator: Proteinuric CKD + hypertension = ACE inhibitor or ARB first-line; BP target <130/80 mmHg.
